Now, you have to fill out the necessary information in the application/complaint form i.e. Details of Complainant (Name, Address, Email Address, Telephone, Mobile number), Details & Location of Complaint (National Highway No., Nearest Place/KM Stone, Prominent Landmark Nearby, City/Town/Village, State), Complaint Category (Complaint Category).

3. After filling the above number of fields, You have to provide comments/suggestions in approx 200 words. You can also attach the photograph if there is a requirement. I am suggesting you to take photograph from your digital camera or mobile phone.
